The Within District Declaration of Residence (WDR) form must be completed by the player first with the receiving association’s registrar. The player must not leave with the form. After completion the receiving association’s registrar will send the form and the proof of residency to the District Registrar.
The receiving association’s Registrar will then request the transfer in the HCR system.
The completed documents and form must be submitted by the receiving association’s Registrar to the District Registrar ASAP.
Upon receipt the District Registrar must verify that all documentation is accurate and complete. He/she will then forward the form (WDR) to the sending association to obtain the remaining 3 signatures.
The sending association’s registrar must sign the bottom part for the Member in Good Standing. The form will not be considered complete otherwise.
The sending association’s registrar then sends the form to the District registrar who will then obtain the signature from the District Chair or Alternate. He/she will then complete the form and accept if it meets all the requirements.
The District Registrar will then upload the forms on the players profile and complete the transaction in the HCR system.
Residency Requirement
Residential purchase document (proves purchase)
Hydro Bill or Gas bill (proves occupancy)
1 year lease agreement, must cover from August 1 to July 31st
Cannot be a sub-let, must be sole resident
Lease residence must be able to contain Parent and child (two rooms)
If leasing a residence from a family member a sworn affidavit indicating they are the sole occupants of the residence must be provided and that they will reside at that residence from August 1st to July 31st.
Valid drivers’ license
Separation Requirements
Legal separation documents must be provided with court sanctioned custody.
Players must provide proof that they are registered in the school district where they intend to play.
The declaration of minor hockey association cannot be changed unless the custody status changes
Appeals
All appeals are to the District where the WDR application was rejected and will include all involved parties present.
