MDA Absence Form

MDA stresses the importance of attendance for each and every dancer!

It is necessary that each time your child misses dance, that an absence form is filled out.

Attendance to dance classes is a critical part in your child’s learning experience, their teammates learning environment, and to prepare our routines for our year-end recital!

If your child must miss a class please fill out the following form to inform our dance instructors of the absence. (This can be done ahead of time for a planned absence)
