NEW PLAYER INFORMATION AND FORMS

If your child has never played hockey before , please complete the following before registering:

1. Create a Spordle Account

3.PARENT DECLARATION FORM
 - This form is only required for participants brand new to Hockey Alberta or participants moving to Alberta from another province. Please complete and put office@sgmha.ca in the Forward Submission section. 
PROOF OF RESIDENCY REQUIREMENTS

4.PLAYER MOVEMENT FORM
 -  This form is required for participants who have moved within Alberta. 

  • Please ensure the forms are completed with both signatures and the reason is checked off.
  • If the reason indicates a "special exception' please attach the exception letter to the transfer
  • SGMHA still requires proof of residency as well. Please send proof of residency in with the completed form.
  • Examples of Proof of Residency are: driver's license, utility bills, tax bills, etc. Please make  sure the proof of residency matches the address and guardians name on the Player Movement Form.

HOCKEY CANADA SHIFT FORWARD - PARENT COURSE
sTARTING FOR THE 2026/2027 SEASON

Parents are required to complete the Hockey Canada Shift Forward program (previously called Respect in Sport Parent).  Certification in the Hockey Canada Shift Forward is valid for up to four seasons and is associated with a May 1 expiry date.  There is a cost to complete this program that is payable by the member.  (Please ensure that you have clicked on the Hockey Canada Shift Forward Parent Program and not the Hockey Canada Shift Forward Activity Leader Program as they are two different programs.  One does not replace the other.)

The Hockey Canada Shift Forward Program is an online certification program designed to protect our youth as well as enhance Hockey Alberta's mandate of providing a safe and fun environment for all of our participants.  It is Canada's leading online bullying, abuse, harassment, and negligence prevention program for parents, coaches, and community leaders.

The Hockey Canada Shift Forward Course helps define a standard of behaviour for all parents/guardians and create a more rewarding, safe and respectful environment for everyone involved.  Parents want to do a great job supporting their kids.  The Hockey Canada Shift Forward Program provides parents with the tools to do just that.

SGMHA HOCKEY FEES FOR THE 2026-2027 SEASONS​​​​​​​ - please note in addition to these BASE fees, teams will require a seed cheque/payment to the team once budgets are formed and voted on at the first team meeting, should extra costs be anticipated. (Excluding the 4-year-old program) 

YOU MUST HAVE YOUR CHILD'S HCR NUMBER TO REGISTERPlease follow the instructions to find your HCR# in your  Spordle Account

  1. Go to the SPORDLE ACCOUNT
  2. Enter your login credentials (username and password) and click Login.
  3. Click on Members
  4. Once in Members,  you will see HCR#s for your players under their names.

All parents need to have a valid Hockey Canada The Shift Forward - Evolving Hockey Culture Certification (Previously Respect In Sport - Parent ) prior to your registration being accepted. Please log into your Spordle account  to check on the certification date.

HOCKEY ALBERTA SHIFT FORWARD INFORMATION

If you do not have a certificate or your's is expired, please complete the course now. YOUR REGISTRATION IS NOT COMPLETE IF THE HOCKEY CANADA SHIFT FORWARD ( PREVIOUS RESPECT IN SPORT - PARENT COURSE) IS NOT VALID.
