making a complaint

Before clicking the forms to make either a Formal Complaint or a Informal Complaint have you done the following:
followed the 24hr rule? 
Have you talked to your team manager? if not resolved, your team coach? if not resolved, then your age group commissioner? 
Please make sure these steps are followed so we the MLMH board can take your complaint seriously and have it delt with in a timely manner. 

"24 Hour Rule" - Unless there is an immediate health and safety concern, players, families and coaching staff are required to wait 24 hours prior to bringing an issue forward. The 24-hour period is a time to logically assess the situation, understand the facts, and afford all parties involved a "cooling off" period. Waiting this time should help with determining if the issue should be brought forward and to do so in a calm, unemotional manner. The protocol is then to come to your team Manager and/ or a coach and we will take care of the issue from there. Parents are not to be contacting the board (commissioner, president, etc.) without following these steps.

An informal complaint is more conversational and suitable for early-stage concerns or direct communication with a coach or team manager.
