Wednesday, July 23 and Tuesday,  July 29: Flag & Tackle Football Paperwork Drop Night | 6:00-7:30 PM at Waltham Boys & Girls Club   

Thursday, July 31: Last day to register for Flag & Tackle Football; Last day to apply for financial need scholarships

Monday, August 4: Flag & Tackle paperwork due; Tackle Football payments due

Monday, August 11: Flag Football payments due

HOW DO I FIND MY PAPERWORK?  Paperwork packets are sent as a PDF attachment with your registration confirmation email. If you can’t find your registration confirmation email, you can also download the packet from your TeamLinkt account or use PDF packet below.

How to Download Your Paperwork from TeamLinkt

  1. Go to: https://app.teamlinkt.com/
  2. Log in using the username and password you created when registering.
    • Note: You must use a web browser (not the TeamLinkt phone app).
  3. In the bottom left corner, hover over your profile circle.
  4. Click My Family.
  5. Click on your athlete’s name.
  6. At the top right, click Registration.
  7. Click View Invoice (you can also make payments here).
  8. Click DOWNLOAD AYF PLAYER PACKAGE.

PAPERWORK CHECKLIST: All Paperwork must be printed single-sided and do not staple. 

  1. Image Release - Minor
  2. Waiver and Release of Liability - Minor
  3. Emergency Medical Treatment, Consent and Information
  4. Participant Page 1 - Cheerleaders must provide an updated photo. Football players will have photos taken in their jersey.
  5. Participant Page 2 
  6. AYF Code of Conduct - Signed by both player and parent.
  7. Concussion Acknowledgement - Signed by both player and parent.
  8. Absentee Form (CHEER only) - Signature required only; do not date.
  9. Copy of 2025 Physical (dated between 1/1/25 and 7/31/25) OR Medical Clearance Form (Page 5) completed by your physician. It must be dated January 1, 2025 or later. 
  10. Birth Certificate (new players only). Original (for us to view) and a Copy (for us to keep).
  11. 2024–2025 Final Report Card (June 2025 with all semesters)

The following forms are not required: Resume Participation Medical Clearance Form and Absentee Form for Football Please ensure all forms are PRINTED SINGLE SIDED, completed in full and signed as indicated. Please do not staple the pages.
