GAME CHANGE FORM

Please use this approval form when a game change is required:

NOTES:

1.   Please discuss with your opposition any game change requests to reach an agreement PRIOR TO submitting this form.
2.  The team requesting the change is to start the process via the LINK above.
3.  Once the Google form is submitted, you will receive a confirmation that your request has been submitted and an approval request will be sent to the opposition team.
4.  Once the opposition team representative has approved the submission, a copy will be forwarded to the NEMHL Scheduler and NEMHL Webmaster to make the change.
5.  Once the change has been completed a confirmation e-mail will be sent to both teams.
