At the start of the 2014-15 season the Fox Chapel Winter League Program implemented a mandatory background screening policy for all coaches for in-house, travel and tournament teams. The background check is thorough, confidential and begins with the winter 2014 season. The background check must be renewed every 5 years. All commissioners, referees and coaches must submit a background check application by 12/1. All other members of the Board of Directors must also be screened. If you have volunteered to coach in the 2014-15 season, or are on the Board of Directors, or are a referee, and you are older than 18 years of age, you can and must complete your Screening here. If you have current child safety clearances for school or other programs, you can submit copies of those clearances.
What type of clearances will I need?
You will need to obtain 3 different clearances.
1. PA State Criminal Background Check (Fee is $22.00)
2. Child Abuse History Clearance (Free every 57 months)
3. FBI History Check (Fee is $24.20)
Where/How do I get my clearances?
1. PA State Criminal Background Check - https://epatch.pa.gov/home
- Select “Submit a New Record Check”
- Fill out the required information
- Any issues please call 1-888-783-7972
2. Child Abuse History Clearance - https://www.compass.state.pa.us/cwis/public/home
- Create an individual account if you don't already have one
- You will be required to provide your past addresses for the past 40 years or so, along with names and relationships of those with whom you have resided with
- Follow the instructions, results are instant. Save a copy for you will need to upload these later.
3. FBI History Check - https://uenroll.identogo.com
- Enter service code 1KG6Zj
- Identogo only sends one time and does not send paper notification
- You get a ONE TIME ONLY download/viewing of the document. Please save it to your computer. You will need to upload it later.
- IMPORTANT: If you've lived in PA for 10+ years you can print out the Volunteer Affidavit Form and have it notarized. You can get this done at any notary. Indiana Township has one on site and it's really cheap.
