Criminal Record Checks

All coaches, volunteers and board members must complete a criminal record check on file with NMSA in the past 5 years. Contact president@nanaimosoftball.ca to arrange to receive the CRC form.

A Screening Disclosure Form must be completed for years that a full Criminal Record Check isn’t required. Contact president@nanaimosoftball.ca to arrange to receive the Screening form.

Both forms will be made available at the pre-season Coaches meeting.
