West Albany H.S. Students must complete 1 & 2 below
South Albany H.S. Students must complete 1, 2 & 3 below
Home School Students must complete 1, 2, 3 & 4 below

1. WAHS Spring Athletic Participation Form (complete by 2/21/25)
2. WAHS Sports Physical form - Good for 2 years 
3. OGLA Transfer Form (Must fill out if you do not attend West Albany H.S., required each year)
4. WAHS Club Home School Eligibility form (Must fill out if you are home schooled, required each year)
