To access your "AYF Player Package" paperwork:
- Search your email inbox for a registration confirmation email (for the 2025 season, not for the LS clinic) from "TeamLinkt" with the subject line "TeamLinkt Registration." There is an embedded link in this email to take you to your player's "AYF Player Package," and this is what you need.
- Or, if you cannot locate the registration email, log in to teamlinkt, click on "My Family," click on the relevant child, click on "Registration," click on "View Invoice" (for the 2025 season), and click "Download the AYF Player Package."
Paperwork completion instructions:
- Please print all documents single-sided and do not staple them.
- In addition to the forms contained in the AYF Player Package, for athletes who did not participate in 2024, we must collect a copy of your child's birth certificate (used by the league to confirm age eligibility). The birth certificate copy is the only document we need to collect that is not part of the AYF Player Package. We previously collected spring report cards for grade eligibility verification at the league level, but are planning to omit report card collection in the first instance for this year.
Participation, Tracking and ID Card:
- Some but not all information is pre-populated. Please double-check your player's date of birth and make any corrections by hand.
- Page 1: For “division of play - team name,” please write your child's fall 2025 school grade.
- Page 1: For flag players, please tape or staple a headshot (chest up is fine) of your athlete in the box on the top right of the first page. Tackle players do not need a photo at this point – we will add it later.
- Page 2: Please add the medical insurance and any other data fields not pre-populated.
- Page 2: Helmet waiver - this only applies for tackle. Note the “Player Initial” field must be initialed by the child participant (or on his or her behalf by their guardian).
AYF Code of Conduct Form:
- Self explanatory; the bottom must be completed and signed by a parent or guardian.
Waiver and Release of Liability - Minor:
- Self explanatory; the bottom must be completed and signed by both (a) a parent or guardian and (b) the child participant (or on his or her behalf by their guardian).
Mild Traumatic Brain Injury (MTBI)/Concussion Annual Statement and Acknowledgment:
- Self explanatory; the bottom must be completed and signed by both (a) a parent or guardian and (b) the child participant (or on his or her behalf by their guardian).
Medical Clearance Form (this one is meddlesome as explained below):
- You have the option of having your pediatrician fill out this form or we can accept the form letter that some pediatricians prefer to provide, however, the form or letter must refer to a 2025 date and, if you're using an alternative form or letter provided by your doctor, it must be dated in 2025 and contain a statement to the effect that your child is in a suitable physical condition to play sports.
- The league will not accept a document from a physical exam performed in 2024 or a prior year even if it indicates it is valid for one year from the date on the form. Your child doesn't need to be seen in 2025, but the doctor's determination that your child is physically fit must be dated in 2025. There is no way around this league requirement.
Emergency Medical Treatment, Consent and Information:
- Self explanatory; complete missing information with careful attention to the family medical insurance and emergency medical information sections, and a parent or guardian signs at the bottom.
Image Release - Minor:
- Self explanatory; the bottom must be completed and signed by a parent or guardian.
Absentee Form - Please omit.
Birth Certificate - Only needed for athletes who did not participate last season; please submit a copy only, not the original, with your AYF Player Package forms. This is the only document we need to collect from you that is not part of the AYF Player Package).
