STEPS FOR ATHLETIC CLEARANCE & TRANSPORTATION FEES
(Required Each School Year for All Athletes)

IMPORTANT: Schedule your physical for a date between June 1–July 26.  Free physicals are available.  Don’t wait until the last minute.  Athletic Physical forms are available for download of June 1^st each year.

The Athletic Clearance process consists of 3 easy steps:

1.  Schedule physical exam.  If you are unable to see your own doctor, you can book a free physical (info on the BHS Athletic Clearance page).    

2.  Download required forms and complete online Athletic Clearance submission by July 26, 2026 (forms and info on the BHS Athletic Clearance page).

3.  Pay Transportation Fee by August 28, 2026.  Do not pay the transportation fee until you make the Program Roster as this amount cannot be refunded. 

*The Athletic Clearance and Transportation Fee process must be completed before any student-athlete can be added to a 6^th Period athletics class.  You must complete all steps before the deadline or your athlete will be unable to participate.*
