Paperwork Info for the 2025 season

All Paperwork must be printed SINGLE SIDED. DO NOT PRINT DOUBLE SIDED

What will you need?

• Birth Certificate Copy- NEW ATHLETES ONLY- Please bring your original birth certifcate AND a copy for us. 
• Physical or Medical Form in packet with a 2025 date. The Medical form is found in your player pack(next bullet). A physical with a 2024 date will not be accepted. Physical or Medical form NEEDS to be turned in before athletes start practice. 
• AYF Player Paperwork Packet-follow directions below to find it. Please use a browser- not the app. EVERY SPOT NEEDS TO BE SIGNED. 

Steps to finding your paperwork download!

• Go to: https://app.teamlinkt.com/ Log in using the username and password you created whenregistering
• In the bottom left corner, hover over your circle
• Click My Family
• 
• Click on your Athlete’s name
• 
• Click Registration on the top Right
• Click View Invoice (you can also pay here as well)
• Click DOWNLOAD AYF PLAYER PACKAGE
