LITTLE LEAGUE® BASEBALL ACCIDENT INSURANCE INFORMATION
If there is an accident / injury, here are the forms. You may download and print them. The league president will need to be notified and copies of these documents will need to be submitted to Little League once they are filled out -
the league president will also have to sign the forms.
