Ministry Leaders,

Please ensure that your Church Roster Form and Merging Church Rosters have been filled out and signed by a church Pastor prior to your first scheduled regular season games.

  1. HACSL Roster Form (Fillable)
    • Only Fill in Names/Numbers of players that attend the Primary Church
    • Have Pastor Sign and Date the Form
    • Ministry Leader Signature Required
  2. HACSL Merging Church Roster Form (Fillable) 
    • Complete this form for each Merging Church (Max of 4)
    • Only fill in Names/Numbers of players that attend this respective church
    • Pastor Signature Required of Merging Church
    • Ministry Leader Signature Required
  3. HACSL Roster Addition Form (Fillable)
    • Use this form to add new players to your team prior to your first scheduled game of each month
    • Players must have attended 2 church services/events or 1 service and a HACSL Worship Night Event in the previous month.
    • Pastor Signature Required
    • Ministry Leader Signature Required
  4. HACSL Minor Waiver Form (Fillable)
    • Must be completed for all players between the ages of 14 and 17.
    • Parent Signature Required
    • Ministry Leader Signature Required
