Save the Date!! KPYFC 2026 Football Paperwork Night!
When: Monday June 22nd
Where: Diplacido- Anderson Field located at 250 North St, Wrentham
Time: 5:30-7:00 pm (drop in anytime between those hours)
All registered football players must submit their paperwork at this time.

The AYF player packet was included in your registration confirmation email from TeamLinkt. If you deleted this, it can also be found in your TeamLinkt account.
REQUIRED DOCUMENTS:

AYF Player Paperwork Packet: This was sent in your TeamLinkt registration confirmation email (please check spam/junk folders). If needed, you can also download it directly from your TeamLinkt account. - See instructions below.

Birth Certificate (NEW athletes only): Must provide ORIGINAL and a COPY - KPYFC will review the original and keep the copy.

Medical Form (REQUIRED): A 2026 dated physical OR a Medical Clearance Form (found in the AYF Player Packet). Must be signed by a pediatrician and dated 2026. These are the only forms accepted; a note from the Doctor will not be approved. This is mandatory — your child cannot participate without this!!

2025–2026 End-of-Year Report Card - if you have not received your report card by 6/22 we will send instructions on how to email it.
IMPORTANT PRINTING & SIGNATURE REQUIREMENTS:

All paperwork MUST be printed single-sided ONLY. (double-sided copies will not be accepted)

All forms MUST be signed by both the parent/guardian AND the athlete where applicable. Please check all paperwork thoroughly for signature/initial requirements.
HOW TO DOWNLOAD & PRINT YOUR AYF PLAYER PACKET (TEAMLINKT):
- Go to: https://app.teamlinkt.com/
- Log in using the username and password you created during registration
- Locate your Family Circle:
- On mobile/app: Top right corner
- On desktop: Bottom left corner
- Click on Manage Family
- Select your Athlete’s Name
- Click Registration
- Click View Invoice
- Select DOWNLOAD AYF PLAYER PACKAGE
- Once downloaded, open the file and print your player packet single sided only!!
- Any paperwork printed double sided will not be accepted!!

If you have any questions regarding registration or paperwork please reach out to kpyfc.reg@gmail.com. If you are unable to attend due to scheduling conflicts, please reach out to your head coach once teams are assigned. Your coach will follow up with you, and you can coordinate directly at that time.
