King Philip Youth Football Paperwork Night!!
When: Monday June 23th (Rain Date Tuesday June 24th)
Where: Diplacido- Anderson Field located at 250 North Street Wrentham MA
Time: 5:30-7:00 pm
All registered football players must submit their paperwork at this time.
*Important information about paperwork, please read carefully*
The AYF player packet was included in your registration confirmation email from TeamLinkt. If you deleted this, it can also be found in your TeamLinkt account.
Steps to finding your paperwork download:

Go to: https://app.teamlinkt.com/ Log in using the username and password you created when registering.

In the bottom left corner, hover over your circle

Click My Family
-
Click on your Athlete’s name

Click Registration on the top Right

Click View Invoice
-
Click DOWNLOAD AYF PLAYER PACKAGE
Please take note on the following:

Paperwork MUST be printed single sided. We are not able to accept double sided printouts.

Signatures: Paperwork MUST be signed by parent/guardian AND child where applicable.

Medical Forms: A 2025 dated medical physical OR Medical Clearance Form (in AYF Player Packet) signed by a pediatrician dated January 2025 or later. This is very important; your child cannot participate without this.

New Players: NEW athletes need to bring ORIGINAL & COPY of their child's birth certificate.

Report Card: Bring a printed copy of the player’s final 2024-2025 report card.
