Player Individual FORM

ALL PLAYERS MUST REGISTER TO PLAY, if you are a permanent member of a team or a call up.  This form must be filled out to appear on a roster and for insurance coverage.

to register on a team that has been registered by a team rep
or
to register as an individual to be placed on a team by the league.
